Olmsted Falls Athletic Boosters

All Olmsted Falls Student-Athletes Entering 7th-12th Grade NEED AN OHSAA PHYSICAL EVALUATION FOR THE 2025 – 2026 SCHOOL YEAR!

When:       Thursday, May 22, 2025  between 3:00-5:00pm

Where:      Southwest General Olmsted Medical Center

                     Bagley Rd. Across from Shakers IGA

Cost:         $30.00.  Cash, Credit Cards, or Check paid to OFAB.  

  

Sign-ups: Please only sign up 1 student-athlete per slot and bring: 

1. Cash/Credit Card/Check  (Check made payable to the OFAB or Cash)

2. Complete Physical forms from Final Forms:

   1.  Go to  Final Forms  and complete the info on Final Forms.

   2. Print out the Physical PPE sheet, ensure it is signed by a parent and bring it with you to Physical Day for the Doctors to complete the exam.
      REPEAT- please print out the exam sheet from FinalForms 

3. Parent MUST sign the form before your physical can be given. 

4. Parent do not need to be present but they must sign  the form.
